2025 Open Enrollment: November 3rd^st – November 17^th, 2025

IMPORTANT FAQS:

If you are currently enrolled in ASI Flex and would like to continue coverage, you must enroll every calendar year. If you do not enroll during Open Enrollment, your current contributions will not continue into 2025.

If you are not making any changes to your LID (STD, LTD, AD&D, and Life Insurance) plan then no action is needed.

If you are not making any changes to your AFLAC policy, then no action is needed.

If you would like to make changes to your beneficiary, please find the form in the LID Insurance Packet below.

Making changes? There are a couple of ways to return your updates….

or the drop box located in the lobby at 5 E. Reed St.

If you would like to make changes to your benefits please complete the required forms below.

If adding a spouse a copy of the marriage certificate and social security card is required.

If adding a child/dependent a copy of the birth certificate and social security card is required.
